Description

Scrub typhus, often known as bush typhus, is caused by the bacterium Orientia tsutsugamushi. It is spread to people through the biting of infected chigger larvae, which mature into mites. 

Scrub Typhus Geographical Distribution

Common Regions:

Scrub typhus is most commonly seen in the following areas:

  • Rural Southeast Asia

  • Indonesia

  • China

  • Japan

  • India

  • Northern Australia

However, scrub typhus was discovered in North Carolina in July 2023, which was a surprise.

Affected Populations And Risk Factors

Scrub typhus can infect people of all ages and genders. Certain circumstances, however, enhance the likelihood of picking up the disease. People are more prone to contract scrub typhus if they come into touch with regions where chiggers thrive, such as rural or forested areas under certain conditions.

Symptoms Of Scrub Typhus

The First Bite:

When an infected chigger bites for the first time, it is frequently painless and passes unreported. However, irritation and the formation of a tiny, reddish welt or sore around the bite occur within a few hours. The bacteria within the body has an incubation period of 6-10 days, with symptoms often appearing 10-12 days following the bite.

Typical Symptoms:

Symptoms of scrub typhus include:

  • Fever

  • Chills

  • Formation of a dark scab known as “eschar” at the bite site

  • Rashes

  • Headaches

  • Muscle pain

  • Confusion

  • Swollen lymph nodes

  • Bloodshot eyes

  • Vomiting

  • Diarrhea

  • Lung infection

Extreme Cases

Individuals in extreme cases may experience problems such as unconsciousness, organ failure, hemorrhage, and more if immediate medical attention is not provided.

Scrub Typhus Remedies

Antibiotics are the most effective scrub typhus treatment. Early treatment is essential for a quicker recovery. Doxycycline is a regularly prescribed antibiotic that can be taken orally or intravenously (IV). To avoid potential negative effects, children may be given a shorter duration of medication.

Complications Of Scrub Typhus

Scrub typhus, if left untreated, can cause significant difficulties in a variety of organs, including the heart, gastrointestinal tract, lungs, kidneys, and central nervous system.

Heart Problems:

  • Disruption of heart rhythm

  • Myocarditis (inflammation of the heart wall)

Issues In The Gut:

  • Vomiting

  • Nausea

  • Diarrhea

  • Liver issues

  • Bleeding in the gastrointestinal tract

  • Pancreatitis

Problem In Breathing:

  • Acute respiratory distress syndrome (ARDS), a life-threatening lung condition

Injury In Kidney:

  • Acute kidney injury (AKI) leading to kidney failure

Neurological Issues:

  • Scrub typhus meningitis

  • Meningoencephalitis

  • Visual hallucinations

  • Nerve pain

Early medical intervention is critical for symptom relief and improved results.

Vaccination Against Scrub Typhus

There is currently no vaccination available to prevent scrub typhus.

Keeping Scrub Typhus At Bay

Safety Precautions:

Individuals should take precautions when exposed to chigger-prone regions, especially while trekking or spending time in woodland areas, to limit their chance of developing scrub typhus.

Precautions include the following:

  • Covering exposed skin with clothing

  • Using insect repellent containing DEET

  • Avoiding direct contact with grass

  • Applying sunscreen after using repellent

  • Protecting babies and children with clothing and protective nets

  • Avoiding direct application of repellent to children under 2 years old (consult a doctor)

  • Washing clothes and gear in hot water and taking a hot shower after exposure

  • Treating outdoor gear with permethrin for added protection (not for use on skin)

By following the above mentioned precautions, people can reduce their risk of scrub typhus infection.
